When a client enters your office, he or she usually is aware of some problem or set of symptoms but is less aware of a goal for treatment. We know that if the client and therapist have a shared clear understanding of the goal of therapy, this with high probability will contribute to treatment outcome. This is true regardless of the treatment model.
A goal gives the therapeutic work a direction and fuels the process with “approach”-motivation instead of “avoid”-motivation. So how can we go about facilitating goal-setting when the patient´s main concern, understandably, is symptoms? I used to think this was basic and easy. But the more clients I´ve seen, the more it has become clear to me that “basic” does not necessarily mean “easy”.
